Seniors' Services

Appropriate health care is necessary for seniors across the continuum: from the many seniors living independently, to those at home requiring minimal support, to those requiring moderate support services, to the frail who require residential long-term care. Achieving this is one of the priorities of the North West LHIN’s Integrated Health Services Plan.

The population in Canada is getting older and the North West LHIN is no exception. The latest numbers from Statistics Canada show 16% of the northwest’s population is 65 years old or older, which is above the national average and up from the last census.

At North West LHIN community engagement sessions across the region, seniors told us they prefer to age at home and want the appropriate services in place to enable them to stay in the community. This concept is getting a lot of consideration and support.

Much has been happening to advance the improvement of health care options, services and systems for our seniors. A number of local initiatives and provincial programs have been announced and are listed below for your reference.

North West LHIN Seniors’ Services Advisory Team

An Advisory Team and Community of Interest for Seniors’ Services has been formed.

Centre of Excellence for Integrated Seniors’ Services 

Input from seniors across the northwest helped initiate the concept for this innovative seniors’ complex, which will provide a complete range of seniors’ services.

Aging at Home Strategy 

The seniors population’s call for more support to stay at home is being heard across Ontario. A three-year Aging at Home Strategy has been announced to help seniors live independently. The North West LHIN is leading this initiative in the northwest.

Flo Collaborative 

This project is designed to improve transitions from acute care hospitals to subsequent care settings for all patients. Two teams in the North West LHIN are involved in this quality improvement collaborative.
