Aboriginal Community Support Services Base Funding Opportunity:
CALL FOR PROPOSALS
The North West Local Health Integration Network is pleased to announce $417,000 in new base funding for 2010/11 which will annualize to $518,000 in 2011/12. The new funding builds on the March 30, 2006 commitment of the McGuinty government to expand Aboriginal community support services.
Types of Services that can be funded under this call for proposals include:
- Meal Delivery
- Social and Congregate Dining
- Transportation
- Homemaking
- Home Maintenance
- Visiting Social and Safety and,
- Service Arrangement Coordination
The North West LHIN call for proposal was issued on July 12, 2010. Proposals should support priorities outlined in the North West LHIN’s Integrated Health Services Plan (IHSP), including reduction in length of hospital stay; decrease of unnecessary admissions to long-term care and hospital; health and safety of seniors; community supports that maintain the independence of seniors within communities across the North West; and increased partnerships and collaborative initiatives.

Please note that evaluations of all proposal submissions will be based on the North West LHIN’s decision-making criteria. These decision-making criteria have been developed in consultation with stakeholders and focus on the following four areas:

- system performance.
