Advancing a CDPM Strategy for the North West LHIN
As health system transformation in Ontario evolves, improvements in chronic disease prevention and management (CDPM) have become a key priority. There is growing awareness of the future challenge to manage demand for health services as the population ages and the needs of chronically ill clients increase. Chronic disease prevention and management was identified as a strategic priority for change in the North West LHIN's Integrated Health Services Plan, 2006.
The North West LHIN has moved forward with a number of initiatives:
- CDPM Advisory Team : An Advisory Team and Community of Interest for Chronic Disease Prevention and Management has been formed.
- Sessions and Workshops : Workshops and continuing professional development sessions are held to build capacity in this area.
- Collection of Articles of Interest: Relevant reports and articles have been reviewed and are available for your reference.
- Community engagement activities have taken place.
The transformation of the health system will take time. By working together, we can achieve our vision of Healthier people, a strong health system - our future.
