The North West Local Health Integration Network, in partnership with the Family Health Team (FHT) conference in Thunder Bay in October 2008, held a session for regional clinicians practicing in interprofessional FHTs on quality primary care relating to chronic disease prevention and management. March 26, 2008 -- Forum: An Opportunity for Real Change: Advancing Health System Transformation in the North West LHIN
The North West LHIN hosted this forum which saw four nationally and internationally recognized speakers engage over 300 of our local health system partners about health system transformation in one place in one day.
The four expert speakers at the forum provided the opportunity to learn from their innovation and integration activities, including:
