Advancing Self Management in the North West LHIN Workshop Series

This skillbuilding and networking workshop is being hosted by the North West LHIN and is for health care providers interested in improving chronic disease management in their practice. There will an emphasis on self management for Aboriginal people.

NORTH WEST LHIN VOLUNTEER OPPORTUNITIES

  • Health Professionals Advisory Committee (HPAC). Collaboration and consultation with our many stakeholders is integral to the mission of the North West LHIN. As front line providers of care, health professionals are essential members of our community and have a valued role in helping to achieve the vision of healthcare in Ontario. Each LHIN is establishing a Health Professionals Advisory Committee comprised of members from a range of health services professions. This multi-disciplinary committee will have the responsibility of providing advice to the network on how to achieve patient-centred health care.
